INFORMED CONSENT FORM

Pursuant to James Madison University Policy I:01:06

IMPORTANT: This sheet should be turned in to your laboratory instructor at the beginning of the first laboratory period.

You should not participate as a subject in any of the exercises utilizing humans if there is any question that you might be adversely affected. Certainly, any students who have a heart, lung, or other physical disability should exercise the same caution that they do on a daily basis. If you do not feel comfortable participating as a subject of an exercise, then you should find some other way to participate in the lab, such as being the recorder or operating an instrument. There is no grade penalty for exercising this option.

I agree to read carefully all exercises involving human subjects before coming to the laboratory. I further agree to use caution and stop immediately any activity with which I am uncomfortable. In addition, I will notify the instructor if injury, unacceptable pain, dizziness, excess fatigue, or other symptoms appear. I am also aware that certain exercises can cause discomfort, adversely affect individuals with existing health problems, and may reveal conditions that have not previously been identified.
